I, the undersigned, parent or legal guardian, acknowledge the inherent risk involved in ice skating, and all sports relating thereto. Accordingly, in consideration of myself, or my child being allowed to participate in any skating activities and/or other activities with Ryan DuPraw & DuPraw Powerskating LLC. on-ice, or off-ice activities, I agree to the following:

I ACKNOWLEDGE AND FULLY UNDERSTAND THAT I (or child) WILL BE ENGAGING IN ACTIVITES THAT INVOLVE RISK OF SERIOUS INJURY WHICH MIGHT RESULT NOT ONLY FROM MY (or child) ACTIONS BUT ALSO FROM THE ACTION, INACTION OR NEGLIGENCE OF OTHERS; AND FURTHER, THAT THERE MAY BE RISKS NOT KNOWN TO ME OR NOT REASONABLY FORESEEABLE.

1. ON BEHALF OF MYSELF (or my child), I AGREE TO ASSUME ALL THE FOREGOING RISKS AND ACCEPT PERSONAL RESPONSIBILITY FOR MY OWN DAMAGES FOLLOWING SUCH INJURY.

ON BEHALF OF MYSELF (or my child), I RELEASE, DISCHARGE, WAIVE AND COVENENT NOT TO SUE DuPraw Powerskating LLC. , AND ALL THEIR RESPECTIVE AGENTS, AFFILIATES, ASSOCIATES, OFFICERS, DIRECTORS, OWNERS, AND EMPLOYEES (COLLECTIVELY “RELEASEES”) FROM DEMANDS, LOSSES OR DAMAGES ON ACCOUNT OF AMY INJURY, DEATH OR DAMAGE TO PROPERTY, CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY RELEASEES OR ANY OTHER PARTYS ACTIONS, INACTION, OR OTHERWISE; AND AGREE TO INDEMNIFY RELEASEES FROM ANY AND ALL THIRD PARTY CLAIMS CAUSED IN WHOLE OR IN PART BY MY (or child) ACTIONS ON OR OFF THE ICE.

Consent to Photograph

As a further condition of my attending, participating, spectating, or volunteering on or off-ice activities with DuPraw Power Skating LLC., I grant DuPraw Power Skating LLC. perpetual and non-revocable permission to use my name, photographs and video in which image, voice and likeness appears in connection with my participation in or spectating activities with DuPraw Power Skating LLC. and further grant permission to display, publish, distribute, use, print and reprint such images, voice and likeness, and the right to employ such images, voice or likeness in advertising and promotions for DuPraw Power Skating LLC., including and advertisements, or media and electronic display and transmission thereof (herein “Likeness Rights”). I release DuPraw Power Skating LLC. from any and all liability for damages for use in any manner or media of the Likeness Rights, and waive any and all claims and causes of action for damages for use of the Likeness Rights including but not limited to unauthorized use of my likeness, image, voice character or persons; violation of my right or privacy; and for copyright or moral rights infringement, defamation, or being cast in a bad light. I understand and agree that this Agreement is a full and final release covering all known and unknown and anticipated injuries, debts, claims or damages to me that have arisen or may have arisen from any matters, acts, omissions or dealing released in this agreement, including but not limited to the use of the Likeness Rights.

I have read the above Waiver and Release, and understand that by signing below, I have given up substantial rights on behalf of myself or my child.
